Heart Failure
MPMG offers a disease management program for patients with heart failure. The goal of this program is to improve patients' self management skills and quality of life. Patients utilize an in-home communication device that records data on key clinical symptoms and self-management behaviors, and provides education and support. Patients who also have diabetes receive self-management education and support for their diabetes. A nurse care manager checks patients’ progress and is available by phone Monday through Friday to answer questions and provide support. Please call 650 240-8108 to learn more about this program.
Diabetes
MPMG offers a disease management program for patients with diabetes. We work with patients and their primary care physician to make sure patients receive all recommended tests and that they have the information and resources they need to control diabetes. We also send reminders when patients are due for tests and periodic mailings with helpful information. MPMG also offers wellness coaching to its members with diabetes. Coaching is a dynamic process that will help patients accomplish their diabetes self care goals. Complex Care Management Program
A qualified team of nurse care managers and care management coordinators are available to assist patients with complex medical needs to obtain an optimal level of wellness and function. In collaboration with your doctor and other members of the healthcare team, MPMG care management staff coordinate, care, services and resources and work with the patient to develop a care plan based on the patient’s goals and needs.
